I certify that the statements, profiles, and images contained in this application are true and complete to the best of my knowledge and understand that (if subsequently employed) falsified or misleading statements on this application shall be grounds for immediate dismissal withoug recourse or further explanation.

I authorize investigation of all statements contained herein and the references and employers listed above to give all information concerning my previous employment and any pertinent information they may have. I furthermore release Tulsa Baking Inc. dba Merritt's Bakery from all liability for any damages that may result from utilization of such information.

I understand that in compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification documentation form upon hire. Merritt's Bakery does participate in the E-Verify system.

I understand that upon, and as a condition of, employment I may be required to sign a confidentiality agreement [containing non-competition, non-disclosure, and confidentiality clauses] and a binding covenantal agreement [with terms on early resignations, work conditions, and exclusivity], that I must have a current food handlers permit (prior to beginning work), and adhere to all aspects of the employee manual, including uniforms which are at my expense, all as required conditions of employment.

I understand that if employed by Merritt's Bakery, it is temporary for six weeks and will be considered as a probationary employment for the purposes of qualifying for a permanent position. Within this period, I may be terminated without cause or justification should it be deemed necessary.

I understand that, if hired, I am obligated to comply with any and all current and subsequently adopted policies, procedures, and efficiency benchmarks adopted by Tulsa Baking Inc. dba Merritt's Bakery. I understand and agree that, if hired, my employment will be on an "at will" basis and my employment  is for no definite period of time. I understand and agree that my employment can be terminated with or without cause and with or without notice at anytime regardless of the date of payment of my wages or salary.

I agree to immediately notify Tulsa Baking Inc. dba Merritt's Bakery if I should be convicted of a felony, or any crime, involving dishonesty, breach of trust, controlled substances, sexual misconduct, abuse, or violence while my job application is pending or during my period of employment.

I understand and consent to, if hired, that Tulsa Baking Inc. dba Merritt's Bakery and its authorized agents shall be entitled, without further consent, to copyright, sell, or use in any manner, any picture, photograph, or recording of my person or voice persuant to my employment at Tulsa Baking Inc. dba Merritt's Bakery.

I understand that as a condition of receiving my final check, I will be required to complete an exit interview. This interview will confirm my status of departure, surrender of any company property in my posession, and the condition of my benefits package along with other details.

I understand that no person is authorized to change any of the terms mentioned in this employment application.
